This service is interdepartmentally based with participants from the Departments of Medicine, Pathology, and Pharmacy. Its purpose is to provide the medical practitioner with relevant information regarding dosing methodology and the development of rational therapeutic drug monitoring schemes.
Interpretation of drug concentration-time data is also available using model- and drug-specific microcomputer software. Drugs included are anticonvulsants (i.e., carbamazepine, phenytoin, phenobarbital), antimicrobials (i.e., aminoglycosides, chloramphenicol, flucytosine, vancomycin), cardiac drugs (i.e., digoxin, lidocaine, procainamide, quinidine), co-trimoxazole, cyclosporine, lithium, salicylates and theophylline.
In addition to the specialized consultations provided on request by the Applied Pharmacokinetics Service, the pharmacy, in conjunction with the clinical chemistry laboratory, provides routine scheduling of selected serum drug samples. Sample requests are screened for necessity and appropriateness by a clinical pharmacist. Then draw times are scheduled to best fit the optimal sampling strategy and laboratory time requirements as well as to avoid in vitro interference by concurrent medications. Presently, only antimicrobial sample requests are routinely screened and scheduled.
